Appointments may be made by calling the office during business hours at (408) 293-JTMG (5864). Web-based self appointment scheduling will soon be available.

     First Appointments

Please bring an ID [drivers license] and insurance card. Forms [available below] can be filled out in advance or at the time of visit. It is our policy that co-payments need to be collected prior to the patient leaving the office. Payments can be made in cash, check or credit [sorry, no Discover Card accepted]

As with all medical offices, patient information forms, financial/insurance information forms and acceptance of our privacy policies will need be to completed before seeing any of our doctors. [Adobe Acrobat is required to view these documents]
