Is there anything I can do to prepare for my first appointment?As with all medical offices, patient information forms, financial/insurance information forms and acceptance of our privacy policies will need be to completed before seeing any of our doctors. Also those interested in reviewing our privacy policies can do so at their leisure prior to their first visit. The forms below can be printed and filled out prior to your appointment. What do I need to bring with me on my first visit?ID [drivers license] and insurance card. It is our policy that copayments need to be collected prior to the patient leaving the office. Where can I park?As part of the Miraido Village there are 17 spots allocated for JTMG patients in the parking lot located behind the practice.
