     What’s the best way to setup an appointment?

Appointments may be made by calling the office during business hours at (408) 293-JTMG (5864). Web-based self appointment scheduling will soon be available.

     Is there anything I can do to prepare for my first appointment?

As with all medical offices, patient information forms, financial/insurance information forms and acceptance of our privacy policies will need be to completed before seeing any of our doctors. Also those interested in reviewing our privacy policies can do so at their leisure prior to their first visit. The forms below can be printed and filled out prior to your appointment.      What do I need to bring with me on my first visit?

ID [drivers license] and insurance card. It is our policy that copayments need to be collected prior to the patient leaving the office.

     What services do you offer?

Pediatrics, Adolescent Medicine, Adult Internal Medicine, Geriatrics, Women's & Men's Health, Minor Surgery, Referral Services and Rehabilitation.

     Where can I park?

As part of the Miraido Village there are 17 spots allocated for JTMG patients in the parking lot located behind the practice.

     What insurance types do you accept?

Accepting Most PPO & HMO (Santa Clara County IPA) Insurances & Medicare. Click here to see a list of insurances that we accept.

     Do you accept self-pay patients?

Yes.

     Will you see patients that have other HMOs such as Kaiser?

Yes, as self-pay status.

     What methods of payment do you accept for co-payments?

Visa, Mastercard,Check and Cash.
